Registration form

To access the application you have to register online.

As DKV client your registration will be processed automatically. After submitting this form you will receive a confirming email containing your activation link, your username and your password. By clicking the activation link once your account will become active and you can log in.

Customer information

You can find your DKV client number on your card in the orange marked area. The 17 digit number in the middle of the card is your DKV card number.

DKV card number: the 17 digit number in the middle of the card.
DKV client number: 6 digits, at the bottom left corner of your card (orange marked area in the example).
DKV card expiry date: 4 digits, at the bottom right corner of your card.
